 Seventeen of 1,139 Tests Positive for COVID-19; 
Declaration Form Available for those Without Secondary Vaccination Verification; COVID-19 Vaccination and Testing Schedule; Food Commodities Distribution Continues
 
Results: Seventeen New Case of COVID-19
Seventeen (17) new cases of COVID-19 were identified out of 1,139 tests performed Friday, June 18 to Sunday, June 20. Two (2) cases were identified through contact tracing. Eight (8) cases reported recent travel history and were identified in quarantine. To date, there have been a total of 8,284 officially reported cases of COVID-19 with 139 deaths, 51 cases in active isolation, and 8,094 not in active isolation. The CAR Score is 0.4.
 
Declaration Form Available for those Without Secondary Vaccination Verification
In line with the Department of Public Health and Social Services (DPHSS) Guidance Memorandum 2020-11 Rev. 12, individuals who are fully vaccinated with a U.S. Food and Drug Administration (US FDA) Emergency Use Authorization (EUA) COVID-19 vaccine, or with a World Health Organization (WHO) Emergency Use Listing (EUL) COVID-19 vaccine, may be exempt from quarantine and must present the following documents upon arrival:
 
  1. Photo identification (e.g. passport, driver’s license, state-issued identification);
  2. Primary COVID-19 vaccination record, which includes the patient name, date of birth, vaccination date(s), and the brand of COVID-19 vaccine administered; AND,
  3. Secondary form of COVID-19 vaccine verification; OR
  4. Declaration of Individual Attesting to COVID-19 Vaccination, signed under penalty of perjury

Individuals who were vaccinated on Guam will not be required to present vaccination verification upon arrival to the A.B. Won Pat International Airport Authority. This service, however, will be made available to the community in the event a secondary verification form is required for entry into other jurisdictions. Availability of the vaccination verification documentation may also streamline the arrival process for passengers seeking entry into Guam.

Requests can be made via phone Monday through Friday from 9 a.m. to 3 p.m. Vaccination verification pick-up times are currently scheduled for Wednesday through Friday from 9 a.m. to 3 p.m. at UOG. 
 
COVID-19 Vaccination Schedule 
COVID-19 vaccination clinics continue at the locations and times below. Bring a photo ID. Those due for their second dose are reminded to bring their COVID-19 immunization card. Minors must be accompanied by a parent or legal guardian and provide government-issued identification to validate age. Minor’s birth certificate is acceptable. Parent/legal guardian must have a government-issued ID. Guardians must also present legal guardianship or power-of-attorney document.

Food Commodities Distribution Continues
The Guam Department of Education (GDOE), State Agency for the Emergency Food Assistance
Program (TEFAP) under the U.S. Department of Agriculture Food Nutrition Service distribution
will be available to village constituents only on a first-come, first-served basis. A limit of three
households per vehicle will be serviced at the distributions. Face masks must be worn. Bring a
photo ID. Village Mayors distribution schedule is as follows:

Free COVID-19 Testing Schedule
COVID-19 community testing continues. No symptoms are needed to qualify for testing. Bring a photo ID. Testing for travel purposes will not be conducted at these community testing sites. Individuals seeking COVID-19 testing to travel off-island may contact the DPHSS Northern Region Community Health Center at 635-7525/6 to schedule an appointment. Priority is given to patients under investigation and those traveling for medical purposes. For the safety of the community and nurses, only four (4) people per vehicle are allowed to be tested. Results available onsite. Testing times, dates and locations are below:
