Individuals must not have a current COVID-19 infection and are encouraged to get tested prior to receiving a COVID-19 vaccine
Individuals may avail of both COVID-19 testing and the COVID-19 vaccine. In those cases, the COVID-19 test will be administered first. Upon receiving a negative result, the COVID-19 vaccine will be made available.
Minors must be accompanied by a parent or legal guardian and provide government-issued identification to validate age. The parent or legal guardian will also be required to sign a consent form on behalf of the minor to be tested or vaccinated for COVID-19. A limit of four participants per vehicle will be observed. Upon arrival, participants must adhere to all traffic directions at the site.

COVID-19 vaccination clinics continue at the locations and times below. Please bring a photo ID. Those due for their second dose are reminded to bring their COVID-19 immunization card.
Verification of COVID-19 Vaccination Available by Request
Individuals who have been fully immunized in Guam with the COVID-19 vaccine can request for verification of their COVID-19 vaccination by making an appointment with the DPHSS Immunization Program. To book an appointment, call 735-7143 or send an email to immunizationprogram@dphss.guam.gov. Requests should be submitted five to seven days in advance. Those who were fully vaccinated at one of the enrolled COVID-19 provider clinics or pharmacies may also make a request for verification of their COVID-19 vaccination through their private healthcare provider.
