Governor Leon Guerrero Signs Executive Order No. 2021-19; UPDATE: COVID-19 Vaccination Schedule; Homebound Vaccination Requests
Today, Governor Lou Leon Guerrero signed Executive Order No. 2021-19, relative to implementing vaccination requirements for certain establishments and activities and limiting social gatherings, and orders such that:
Places of worship may continue services, ceremonies, and operations as provided in Executive Order No. 2021-16, but must strictly comply with requirements provided in applicable DPHSS guidance, including mask-wearing requirements.

UPDATE: COVID-19 Vaccination Schedule
COVID-19 vaccination clinics continue at the locations and times below:
Bring a photo ID. Second dose recipients should bring their COVID-19 immunization card. For the complete schedule visit dphss.guam.gov/vaccinategu.
Special Requirements for Minors to Receive the Pfizer-BioNTech Vaccine:
Homebound Vaccination Requests
DPHSS and the University of Guam (UOG) Center for Excellence in Developmental Disabilities Education, Research, and Service (CEDDERS) have partnered to service all homebound requests. Individuals who would like to request homebound COVID-19 vaccinations should call 311, option 2, between 8 a.m. to 5 p.m. Monday to Friday, to schedule an appointment.

Governor Leon Guerrero Signs Executive Order No. 2021-19; UPDATE: COVID-19 Vaccination Schedule; Homebound Vaccination Requests
Today, Governor Lou Leon Guerrero signed Executive Order No. 2021-19, relative to implementing vaccination requirements for certain establishments and activities and limiting social gatherings, and orders such that:

Proof of vaccination is required for patrons 12 years of age and older prior to entering all restaurants, bars, clubs, gyms, fitness centers, dance studios, movie theaters, food courts at shopping establishments, bowling alleys, sporting events, concerts, boat cruises, and other establishments and events that may be identified in applicable DPHSS guidance.The requirement for staff to provide proof of vaccination will be in effect at 8:00 AM on September 27, 2021.
- While the vaccination mandate for establishments for entrance of patrons goes into effect on Monday, August 23, enforcement will commence at least two weeks from the effective date, which will be on Monday, September 6.

Acceptable forms of vaccination verification include:
- the CDC vaccination card, or similar documentation issued by another foreign governmental jurisdiction;
- a photo or copy of a vaccination card as a separate document;
- a photo of a vaccination card stored on a phone or electronic device;
- documentation of vaccination from a healthcare provider;
- printed WEB-IZ documentation of vaccination information; or
- other proof as provided in applicable DPHSS guidance memoranda.
- All organized contact sports and competitions must require all participants and coaches to show proof that they are fully vaccinated prior to every event. The requirement for staff to provide proof of vaccination will be in effect at 8:00 AM on September 27, 2021.
- Social gatherings or congregations shall be limited to no more than one hundred (100) vaccinated persons, unless otherwise provided in applicable DPHSS guidance. Unvaccinated persons ages twelve (12) and older may not participate in social gatherings with individuals who are not members of their household. Children under the age of twelve (12) may attend social gatherings.
- Mask-mandate remains in place
- Guam continues to be in Pandemic Condition of Readiness (PCOR) 3

- must provide government-issued identification to validate minor’s age
- must provide minor’s birth certificate
- parent/legal guardian must have a government-issued ID
- guardians must present legal guardianship or power-of-attorney documents
