Guidelines for GSYEP Childcare Applications

PRESS RELEASE

The Department of Youth Affairs (DYA) offers guidance to applicants and their parents or guardians for the Governor’s Summer Youth Employment Program (GSYEP) childcare component. For prompt processing, follow these instructions when completing your application packet.
 
Application Period and Submission
 
  • Applications accepted April 17–24, 2026 (weekdays only).
  • Submit in person at the Youth Center where the application was picked up: Håya, Kåttan, or Lågu Youth Centers. Centers are open Monday–Friday, 8:00 a.m.–5:00 p.m.
 
Required Forms and Documentation All application packets must be complete and include:
 
  1. Completed 2026 GSYEP application form.
  2. Applicant photo ID (e.g., Guam ID, driver’s license, passport, or recent student ID) — copy enlarged to 8.5" x 11".
  3. Copy of Applicant’s birth certificate.
  4. Copy of Applicant’s Social Security Card or Taxpayer ID — copy enlarged to 8.5" x 11".
  5. Copy of parent/guardian photo ID — copy enlarged to 8.5" x 11".
  6. Completed BCCS Consent for Disclosure of Client Information form.
  7. Completed Guam Police Department Authorization form.
  8. Completed Authorization for Certificate of Search (Court Clearance) form.
  9. Completed Health Certificate Authorization Letter form.
  10. Copy of the completed Health Certificate Application (see Health Certificate instructions below).
 
Health Certificate Instructions
 
  • Applicants and parent/guardian must complete page 1 of the Health Certificate Application before obtaining a physical and PPD/TB test.
  • For first-time institutional applicants, mark “INSTITUTIONAL” and “NEW APPLICANT.” For renewals, mark “INSTITUTIONAL” and “RENEWAL APPLICANT.”
  • Pages 2 and 4 must be completed and signed by the medical professional who performs the physical and PPD/TB test.
  • DYA requests a copy of the completed Health Certificate Application with your packet. The Department of Public Health and Social Services (DPHSS) Division of Environmental Health (DEH) requires the original document with the physician’s original signature; DYA will notify selected applicants when to submit the original to DEH.
 
 
Physical exams and PPD/TB Testing
 
DPHSS will not provide the physical exams and PPD/TB testing originally scheduled for Saturday, March 28, 2026, for GSYEP childcare applicants. This service was intended to assist applicants experiencing insurance coverage issues.
 
Applicants with insurance coverage concerns should monitor local news outlets and DYA’s social media channels for updates on rescheduled dates, times, and locations where DPHSS will provide the required physical exam and PPD/TB testing for the GSYEP childcare application.
